This questionnaire is confidential. The facilitator needs this background information to keep all group members safe during the training. It won’t be shared with third parties. Kept on file only as needed, then deleted within 12 months.

4. Agreements

This is an educational training, not medical treatment.

Before you register and pay: you can withdraw within 14 days for a full refund. After that, cancelling more than 30 days before the start costs a €100 registration fee; within 30 days the €300 deposit is kept and other payments are moved to a later training. If we can fill your place or our costs are lower, we refund proportionally. Full cancellation terms
